Child and Youth Registration Deadline April 14th!
Adult Registration with Meals April 21!
Walk-in Registration (no Childcare, no Meals) Accepted.
Please note: you will need

Adult Registration
Please submit one
Adult Registration form
for each adult attending DA.
Child Registration
For each child (6 mos. -11 yrs.), please submit a
Child Registration/Release form
with the Adult registration form of the Parent/Guardian.
Youth Registration
For each youth (12 yrs. - 18 yrs.), please submit a
Youth Registration / Sponsor form
with the Adult registration form of the Sponsor.
All Youth attending District Assembly MUST have an Adult Sponsor 25
years of age or older!
