and then go to PayPal to finalize your registration!
Adult Registration
Please submit one
Adult Registration form
for each adult attending DA.
Child and Youth Registration
For each participant under the age of 18,
please submit a Child / Youth Registration/ Medical Release form
with the Adult registration form of the Parent/Guardian.
All Youth attending District Assembly MUST have an Adult Sponsor 25
years of age or older!
